A decade ago, several patients were considered unfit to go for full mouth dental implants as they had inadequate bone either in the upper jaw or in the lower jaw. Also during this time choices of the implants were very limited, due to which most patients had to finally settle for the old fashioned removable full dentures.
During this time complex bone grafting techniques were available to replace missing bone. Bone piece (graft) was taken from the patient’s ileac crest or mandible and grafted onto the area where there is insufficient bone. There were 2 disadvantages to this bone grafting procedure. One reason being, Implants could be placed into this grafted bone only 6 months later. Other reason being, two simultaneous separate surgical procedures had to be performed on the same patient to take graft material from donor area and transfer to the recipient area. This incurred greater cost of treatment to the patient, more trauma and risk of infection.
New technologies and new techniques have evolved since a decade. There are many graftless solutions available when there is no bone either in the maxilla or in the mandible. The new technologies are the All on 4 implant concept, Zygoma implants and Pterygoid implants supported by some Mini implants.
All-on-4 concept
The All-on-4 dental implant technique is a revolutionary approach that allows for the replacement of 12 to 14 teeth in one arch by utilizing just four implants. This technique is particularly useful for patients who have sufficient bone left only in the anterior segment of the jaws. Four implants are strategically placed in the anterior jawbone, with 2 straight implants in the center and the longer posterior implants angled backwards to maximize contact with available bone. Fixed temporary dentures are constructed by the 3rd day of implants placement. Six months later after osseo-integration, fixed permanent dentures are constructed. Modifications to this technique are the All-on-5 or the All-on-6 where additional implants are placed in the anterior segment in areas where more bone is present. This provided greater support for the prosthesis and greater long term success.
Zygoma Implants

Zygoma implants are longer implants that anchor into the zygomatic bone (cheek bone) rather than the maxillary bone. In situations where entire maxillary bone is lost in the front and the back, Zygoma implants technique is advocated. 2 long implants one on either side are placed through the maxillary bone into the cheek bone through an intra-oral approach. A prosthesis is attached to these implants, either immediately or after a healing period. Modification to this technique is by placing 2 long zygomatic implants on each side instead of 1 long zygomatic implant for greater support for the prosthesis and this is called the Quad Zygoma. Specialized surgical skills are required for performing this procedure.
Pterygoid Implants

Pterygoid implant is another alternative where there is insufficient bone in the posterior maxilla. These implants are placed in the pterygoid process of the sphenoid bone one on either side, Few Mini implants may also be placed in the maxillary bone anteriorly to increase the support for the denture prosthesis. Finally a prosthesis is attached to the implants, either immediately or after a healing period. This procedure requires specialized surgical skills.
